Intake Paperwork

Please take a few minutes to print and fill out our intake paperwork and bring it with you to your first appointment. You are not required to print the notice of privacy practices, but you will be asked to sign that you've received a copy on the last page of the intake paperwork. You may complete the paperwork in our office, however you will need to come in about 10 minutes early to do so.

Our Rice Village Office

Our Rice Village office is in the Lone Star Building at the southeast corner of Kirby and Bissonnet, just two blocks south of Highway 59.

Our Northwest Houston Office

Office Location

Our Northwest Houston office is in the Nanes Professional Building near the northeast corner of Nanes and FM 1960, one mile west of I-45.
